Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/23/25 and completed on 9/23/25, as a part of the licensure period. The on-site inspection began at 9:05am and ended at 10:03am. The inspector reviewed compliance in the areas listed above. There were fifteen children present and three staff. The inspector reviewed five children?s records and three staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The required first aid kits must include all the minimum required items.

The first aid kit was missing triangular bandages.
Plan of Correction: Will order two triangular bandages for the first aid kit.
Standard 8VAC20-780-60-A
Each center must maintain and keep at the center a separate record for each child enrolled that contains the name, address, and phone number of two designated people to call in an
emergency if a parent cannot be reached.

The record for Child #4 was missing the complete address for 1 of 2 emergency contacts.
Plan of Correction: Will obtain the complete address from the parent today.
