Inspector Notes

An investigation was initiated on 03/06/2023 in response to a self-reported incident regarding supervision received on 03/03/2023. The investigation concluded on 3/30/2023. Based on staff interviews and record review, the preponderance of evidence supports a violation and the area of non-compliance is identified on the violation notice. Based on staff interview and record review, one child age 10 or older was not within actual sight and sound supervision.

Evidence:
1. On 3/2/2023, a 12 year old child was left unsupervised outside on the playground for approximately 5 minutes.
2. Staff #1 reports the child was present at the back of the line to re-enter the building. Staff #1 witnessed the child drop a basketball and get out of the line to retrieve the ball.
3. Staff #1 reports the staff and children return to the classroom in a span of 3 to 5 minutes.
4. Upon arriving to the classroom, Staff #1 reports becoming immediately aware that the child is not in the classroom with the rest of the children. Staff #1 returned to the playground where she found the child standing against the wall of the school building.
2. The playground is not located in an area that prevents individuals who are not staff members or persons allowed to pick up children in care from entering the enter the area where the child was unsupervised.
Plan of Correction: The regional supervisor reported the incident to all required parties. The regional supervisor followed up with staff after the incident was reported by staff, reviewed the four part accountability plan, and discussed accountability before transitions and after transitions to ensure safety and accountability of all children in their care. The regional supervisor also initiated an active supervision training with all staff to include reviewing positioning of staff as a part of active supervision.
