Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 3 pm and 6pm. There were 51 children directly supervised by 5 staff. The physical plant, 6 staff records, 5 children?s records, medications, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed having snack, completing homework, and playing outside. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not obtain documentation that each child has received immunizations before the child can attend the center.

Evidence:
1. Child #1 began attending the center on 2/16/2023.
2. The record for Child #1 did not contain documentation of immunizations.
Plan of Correction: We will request the information from the parents.
Standard 8VAC20-780-140-A
Based on record review, the center did not obtain documentation that each child has a physical within 30 days after the first day of attendance.

Evidence:
1. Child #1 began attending the center on 2/16/2023. The record for Child #1 did not contain documentation of a physical exam.
2. Child #2 began attending the center on 8/22/2022. The record for Child #2 did not contain documentation of a physical exam.
Plan of Correction: We have requested the document from the front office. We will request again, or ask for a copy from the parents.
