Inspector Notes
An unannounced monitoring inspection was conducted today from 1:00pm-3:00pm. There were 53 children directly supervised by 6 staff. The physical plant, 6 staff records, 5 children?s records, medications, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed eating snack and playing games. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

4Evidence:
1. Medication in the center for Child #8 had an authorization dated 8/21/2023. The authorization did not contain a signature from the physician. The authorization expired 10 business days from 8/21/2023.
2. Medication in the center for Child #11 had an authorization dated 8/24/2023. The authorization did not contain a signature from the physician. The authorization expired 10 business days from 8/24/2023.
Evidence:
Documentation of emergency drills completed prior to 8/2023 were not available for review.
Evidence:
1. An injury report completed for Child #6 on 1/22/2024 did not contain a combination of staff and parent signatures or two staff signatures as required. The report only contained the signature of one staff.
Evidence:
1. The record for Staff #6 did not contain the address of a person to be notified in an emergency which shall be kept at the center.
