Inspector Notes

An unannounced monitoring inspection was conducted today from 1:00pm-3:00pm. There were 53 children directly supervised by 6 staff. The physical plant, 6 staff records, 5 children?s records, medications, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed eating snack and playing games. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-780-510-P
Based on medication and medication documentation review, the center did not ensure that when an authorization for medication expired, the parent be notified that the medication needed to be picked up within 14 days or the medication was disposed of by the center.
Evidence:
1. Medication in the center for Child #8 had an authorization dated 8/21/2023. The authorization did not contain a signature from the physician. The authorization expired 10 business days from 8/21/2023.
2. Medication in the center for Child #11 had an authorization dated 8/24/2023. The authorization did not contain a signature from the physician. The authorization expired 10 business days from 8/24/2023.
Plan of Correction: Parents will be notified of the expired authorizations.

Based on record review, practice drills were not maintained for one year.

Evidence:
Documentation of emergency drills completed prior to 8/2023 were not available for review.
Plan of Correction: The center will make a copy of the drill record at the end of the year to keep at the center.

Based on record review, 1 injury report did not contain all required components.

Evidence:
1. An injury report completed for Child #6 on 1/22/2024 did not contain a combination of staff and parent signatures or two staff signatures as required. The report only contained the signature of one staff.
Plan of Correction: The center will ensure all required information is completed for injury reports.

Based on record review, the staff record for 1 of 6 staff did not contain all required information.

Evidence:
1. The record for Staff #6 did not contain the address of a person to be notified in an emergency which shall be kept at the center.
