During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site (monitoring) inspection was initiated and completed on 06/03/2025. The on-site inspection began at 3 pm and ended at 4:30 pm. The inspector reviewed compliance in the areas listed above. There were (47) children present and (5) staff. The inspector reviewed (5) children?s records and (5) staff records (documentation required to be on-site) on 06/03/2025.

This inspection included: document review (i.e. Files, injury logs (none), policies and procedures, emergency drill logs, medication administration logs, authorization forms); tour of the facility; interviews; and observations.

Information gathered during the inspection determined a non-compliance with applicable standard, and was documented on the violation notice issued to the program.

The center's procedures for administering medication shall include methods to prevent use of outdated medication.

The two medications being stored on site for Child #1 displayed expiration dates that indicated that they were both expired.
Plan of Correction: Both medications shall be returned, and new medications will be obtained.
