During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An announced review of staff personnel files was conducted on 1/8/2026 and completed on 1/8/2026 by inspectors Diana DeSilva, Miranda Wright, Ariel Hayes and Margaret Allworth at the Pennino SACC office building. The inspection began at 9:30 am and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. The inspection included document review.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Staff #1, who has been actively working at the center for the past year, did not have any completed reference verifications on file.
