An unannounced monitoring inspection was initiated and completed on 01/22/2026, in response to a self-report received by the licensing office on 01/15/2026 relating to standard # 8VAC20.780.400.A (Behavioral Guidance). The inspector reviewed (2) children?s records on-site on 01/22/2026. All staff records were reviewed on-site on a previous date.

This inspection included: document review (i.e. Files, policies and procedures, and written statements), tour of the facility, interviews, and observations.
