Inspector Notes

An unannounced, on-site renewal inspection was initiated on January 29, 2025 and completed on January 29, 2025, as a part of the licensure period. The on-site inspection began at 10:55am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There
were 37 children present and two staff. The inspector reviewed five children?s records and two staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The required ratio for children school age eligible up to nine years is one staff per eighteen children.

Staff #1 was observed alone in the gym with twenty one children (youngest child five years old) from approximately 10:55 a.m. to 11:30 a.m. Staff #2 was observed alone in the classroom with seventeen children from approximately 10:55 a.m. to 11:30 a.m.
Plan of Correction: Will follow the staff to children ratio requirements in the future.
