Inspector Notes
An unannounced monitoring inspection was initiated on 10/15/25 and completed on 10/15/25 in response to a self-report received by the licensing office on 10/9/25 regarding injury reporting. The inspector reviewed one child's record. This inspection included document review, a tour of the facility and interviews.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
