Inspector Notes
A subsidy health and safety (SHSI) inspection was conducted on February 1, 2023. There were 54 children present with 4 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Based on review of accident/incident's reports, the center did not inform the department's representative as soon as possible, but not more than two business days, of any injury to a child while under the center's supervision when a referral is made for treatment from a medical professional.

Plan of Correction: We contacted our supervisor on 1/26/2023 and again on 12/27/2023 to inform them of the incident. We submitted the information within 2 business days to our supervisor.
