During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 7/17/2025 and completed on 7/17/2025 in response to a self-report received by the licensing office on 7/9/25 relating to the areas of the standards regarding Staffing and Supervision and Special Care Provisions and Emergencies.
The inspector reviewed one child's record and three staff records on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/28/25. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION:

When staff are supervising children, they must always ensure their care, protection, and guidance.

On July 8, 2025, during a field trip, Child A's prescription medication was stored unlocked in a backpack under Staff #1's seat. The backpack was left unattended by Staff #1 at times while they assisted other children and was accessible to children in the group.

Staff #3 did not count or verify that all children's medications, including Child A's medication, were returned and placed back into the locked medication storage box at the center upon returning from the field trip. On July 9, when Staff #2 attempted to administer the medication, it was discovered to be missing and was reported lost.
Plan of Correction: While supervising children, staff will ensure their care, protection and guidance at all times. While on field trips the designated staff will maintain possession of the backpack at all times and ensure that the backpack is inaccessible to children.
A field trip medication inventory sheet was designed by the site supervisor which will be implemented as part of the center's field trip procedures. The inventory sheet requires medication to be checked out and back in upon return to the center.

Medication, including refrigerated medication and staff's personal medication, must be kept in a locked place using a safe locking method that prevents access by children.

On July 8, 2025, during a field trip, Child A's medication was stored in a Ziploc bag inside a backpack without a lock. Staff #1 placed the backpack under their seat and at times left it unattended and accessible to children while assisting students. The medication was reported lost by the center on July 9, 2025.
Plan of Correction: While on field trips medication will be kept in a locked safety sack or backpack. Staff will ensure the bag is closed and locked at all times to prevent contents from getting lost. The designated staff will maintain possession of the backpack at
all times and ensure that the backpack is inaccessible to children.

Centers must keep a record of medication given to children including any medication errors.

On July 9, 2025, Staff # 2 attempted to administer medication to Child A but was unable to locate the medication, and reported it lost by the center. The center did not record this medication error on the medication log maintained on site as required.
Plan of Correction: Staff will document any medication errors on the center's medication log
which is maintained in the medication administration notebook.
