Inspector Notes

An unannounced monitoring inspection was conducted on September 26, 2022. There were 13 children present with 3 staff members. The physical plant, menu's, medications and authorizations, staff records, children's records, evacuation logs, attendance logs and the first aid kit were reviewed. The children were observed playing table games and jewelry making. Areas of non-compliance were cited on the violation notice. Based on a review of children's records, the center failed to obtain all required documents for each child enrolled.

Evidence:
Child #3's record does not contain documentation of a place of employment for either parents and one parent does not have a work phone number.
Plan of Correction: I will get this information from the parents.

Based on a review of staff records, the center failed to obtain all required documentation.

Evidence:
Staff #3 did not have any documentation of employment.
Plan of Correction: I will get the staff member's records from the central office.
