Inspector Notes

An unannounced monitoring inspection was conducted on February 28, 2023 from 8:00 am until 9:10 am. There were 18 children present with 3 staff supervising. The physical plant, medications and authorizations, menu's, first aid kits, children's record and staff records were reviewed. The children were observed during check in and playing floor games. Based on review of children's records, the center failed to obtain required documents for all children enrolled.

Evidence:
Child #1, #3, #4 and #5's record did not contain the address of each custodial parent's place of employment.
