Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on October 3, 2024. The on-site inspection began at 9:40am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 30 children present and 3 staff. The inspector reviewed 5 children?s records and 3 staff records onsite. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 11, 2024. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center, inside, shall be maintained in a clean, safe and operable condition.

A chair in the main classroom was cracked on the seat in two places.
Plan of Correction: Chair was immediately removed from classroom.

Staff shall request at least annually parent confirmation that the required information in the child's record is up to date.

The record for child #4, enrolled for 2 years, does not contain parent confirmation that the child's record is up to date.
Plan of Correction: Ensure parent has updated child's file.

The center shall implement a monthly practice evacuation drill.

The center did not implement a monthly practice evacuation drill for the month of August 2024.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
