Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 9:00am-10:00am. There were a total of nine children with three staff members present. Children were observed during breakfast time, handwashing, engaged in group activities and outdoor play time. A selection of staff and children records and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. Child #2 and Child #3's records did not contain documentation of immunizations.
Plan of Correction: Documentation of the immunizations will be obtained by the parents.
Standard 8VAC20-780-140-A
Based on record review, each child did not have a physical examination by or under the direction of a physician.
Evidence:
1. Child #1's record is missing documentation of a physical examination.
2. Child #4's record is missing documentation of a physical examination.
Plan of Correction: Documentation of the physicals will be obtained by the parents.
