Inspector Notes

An unannounced monitoring inspection was conducted on April 21, 2023. There were 26 children directly supervised by 2 staff. The physical plant, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play and reading in various places in the classroom. Areas of non-compliance were cited on the violation notice. Based on a review of children's records, the center failed to obtain all required documentation for each child enrolled.

Evidence:
Child #5's record does not contain documentation of addresses for emergency contacts.
Plan of Correction: I will get the contact information from the parents and place in the child's file.
