Inspector Notes

An unannounced renewal inspection was conducted today between the hours of 7:15am and 8:20am. 12 children were observed directly supervised by 3 staff. The physical plant, 3 staff records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing cards and games, and coloring. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not obtain documentation that one child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
1. Child #1 began attending the center on 2/1/2024.
2. The record for Child #1 did not contain documentation of immunizations.
Plan of Correction: The center will speak with the parent and contact the school to request the documentation.

Based on record review, the center did not ensure that one child had a physical examination within 30 days after the first day of attendance.
