Inspector Notes

An unannounced renewal inspection was conducted today from 3:45pm- 5:54pm. There were 27 children directly supervised by 2 staff. The physical plant, 2 staff records, 3 children?s records, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed handwashing, eating snack, reading books, and playing outside. There were no medications on site. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 2 of 3 children's records reviewed did not contain all required information.

Evidence:
1. The record for Child #1 did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. The child's parents were listed as the emergency contacts.
2. The record for Child #2 did not contain the address for 1 of 2 required emergency contacts.
Plan of Correction: I will get the information from the parents.
