Inspector Notes

An unannounced monitoring inspection was conducted today from 4pm-5:30pm. There were 28 children directly supervised by 3 staff. The physical plant, 3 staff records, 3 children?s records, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed reading, ocmpleting home work, making crafts, and having snack. Handwashing was also observed.
Areas of non-compliance are identified in the Violation Notice.

Based on record review, shelter in place procedures were not practiced a minimum of twice per year.

Evidence:
1. The most recent shelter in place drill was dated 9/24/2021.
Plan of Correction: A shelter in place drill was completed on May 5. I will make sure all drills are documented when completed.
Standard 8VAC20-780-60-A
Based on record review, 2 of 3 children's records reviewed did not contain all required information.

Evidence:
1. The record for Child #1 did not contain the phone number for the child's mother and the phone number for the mother's place of employment.
2. The record for Child #2 did not contain the phone number for the father's place of employment.
Plan of Correction: I will ask the parents for the information.
