The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1, who had been attending for more than six weeks, did not have documentation of an immunization record on file.
Plan of Correction: The main office was closed during the inspection. The center will request the information from both the school and the parents.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record for Child #1, who had been enrolled for more than six weeks. There was no physical examination in the record for Child #2 who had been enrolled for more than one year.

Centers are required to maintain compliance with the center's own policies and procedures required by these standards. The center did not follow its policy regarding methods to prevent use of outdated medication. Medication for Child #2 had been expired for more than two months. Medication for Child #1 and #4 had been expired for more than two weeks.

Shelter in place procedures shall be practiced a minimum of twice per year. The center only practiced one shelter in place drill during the previous school year. The last shelter in place drill was completed one year ago.
