Inspector Notes

An unannounced, on-site renewal inspection was initiated on 10/11/2024 and completed on 10/11/2024. The on-site inspection began at 8:15am and ended at 9:25am. The inspector reviewed compliance in the areas listed above. There were 25 children present and 2 staff. The inspector reviewed 5 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Records on staff shall be maintained and made accessible.

Records for Staff #1 were not accessible during the inspection.
Plan of Correction: Center will ensure that staff records are accessible.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child #1, #2 and #4, enrolled for more than 4 weeks, was missing the work phone numbers of each parent who has custody.
Plan of Correction: Center will obtain the work phone numbers of the parents.
