During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 1/29/2025 and completed on 1/25/2025. The on-site inspection began at 2:25pm and ended at 3:25pm. The inspector reviewed compliance in the areas listed above. There were 56 children present and three staff. The inspector reviewed five children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated10/11/2024, were not posted in a conspicuous place on the premises.
Plan of Correction: The most recent inspection notice was posted during the inspection.
Standard 8VAC20-780-430-K
The center shall have an individual place for each child's personal belongings. The center did not have enough individual places for each child's personal belongings, resulting in many children's belongings being left on the floor.
Plan of Correction: The center will purchase extra equipment so that each child has a place for their personal belongings.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #2, and Child #4, was missing the following required components:
Child #2- missing work phone numbers for both parents, and missing a 2nd emergency contact.
Child #4- missing work phone number for one parent.
Plan of Correction: The center will obtain the missing information for both children.
