Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/10/2025. The on-site inspection began at 2:30pm and ended at 3:30pm. The inspector reviewed compliance in the areas listed above. There were twenty-seven children present and three staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-I
The center administers drugs according to the prescriber's instructions pertaining to dosage, frequency, and manner of administration. The center could not follow the prescriber's instructions because one of two drugs listed in the allergy action plan for Child #3 was not available at the center.
Plan of Correction: The center will obtain the medication from the parent.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #4, was missing the work phone number of one parent who has custody.
