63.2 Child Abuse & Neglect
Inspector Notes
A monitoring inspection was conducted on 5/23/2023 at 8:30am in response to a self reported incident where an eight year old child was left unattended on the playground on 5/17/2023.
Interviews were conducted with staff and children and staff records were reviewed.
Areas of non-compliance are identified in the Violation Notice.

Violations
2Evidence:
1. On May 17, 2023, 8 children were playing outside on the playground. Staff #1 lined up 8 children (including Child #1) at Door 7 to go inside at approximately 8:20am.
2. Staff #1 was unaware that Door 7 closed prior to Child #1 gaining entry to the building and locking Child #1 outside.
2. Upon entering the building the children proceed directly to the bathroom to wash their hands, after which the teacher ensures the presence of all assigned children.
3. While the children assigned to Staff #1 were washing their hands, Staff #2 observed Child #1 run past the SACC classroom window and ring the doorbell at the SACC entrance to gain entry back into the SACC program.
3. Child #1 was admitted back into the building and reunited with program staff approximately 1-2 minutes after being separated from the group.
