Inspector Notes

An unannounced monitoring inspection took place on 06/03/2024 from 3:18 p.m. to 3:58 p.m. There were a total of 28 children supervised by two staff members. The children were observed arriving to the program, washing hands, eating snack, engaged in various activities including puzzles, reading , STEM activities and painting. Positive interaction between staff and children were observed. A complete inspection of the physical plant, 5 children records, 3 staff records, emergency drill logs and emergency supplies were reviewed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. The center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
Child #5's(DOE: 8/21/2023) file did not have documentation of an immunization record.
Plan of Correction: The center will call the office to obtain the immunization record today.
