Inspector Notes

An unannounced renewal inspection was conducted on 3/5/2024 from 11:30 am to 12:05 pm. There were 29 children supervised by 3 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing outside. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on review of four children's records, the facility did not obtain documentation of immunizations for a child prior to the child's first attendance.

Evidence: There was no documentation of an immunization record for Child #2. Child #2's start date is 2/1/2024.
