Inspector Notes

An investigation in response to a self-reported supervision incident that took place during summer camp was initiated on July 12, 2023 and concluded on August 1, 2023. An unannounced inspection was conducted on July 18, 2023 between the hours of approximately 1:30 p.m. and 2:30 p.m. Interviews were conducted, records and center policies were reviewed. Information gathered during the inspection supported the self-report of non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-340-F
Based on staff interview, a child under 10 years of age was not within actual sight and sound supervision of staff at all times.

Evidence: On 07/12/2023, at approximately 9:35 a.m. staff and children assigned to the Athletes? Cabin transitioned from Classroom #216 to the gym. Approximately 2 minutes later, Staff #2 entered Classroom #216, finding Child #1 (age: 5 years) sitting at a desk and playing with a flashlight, unsupervised.

Standard 8VAC20-780-40-E
Based on staff interview and policy review, staff failed to follow the center?s own policies and procedures on the 4-Part Accountability System and Transitions.

Evidence:
1. The center has a policy called the 4-Part Accountability System. Step 4 of the policy states ?each teacher will account for the children in their care on their checklist before they transition.?
2. The center?s policy on Transitions states that when transitions occur, one staff member should ?make sure that all of the children reach the new area and none are left behind.?
3. On 07/12/2023, at approximately 9:35 a.m. staff and children assigned to the Athletes? Cabin transitioned from Classroom #216 to the gym. Approximately 2 minutes later, Staff #2 entered Classroom #216, finding Child #1 (age: 5 years) sitting at a desk and playing with a flashlight, unsupervised.
4. Staff #1 stated that they called the children by grades, not individually, and did not do a face-to-name check using their checklist prior to transitioning from the classroom to the gym.
5. Staff #1 confirmed that Staff #3 and Staff #4 were in the classroom at the time and neither staff member conducted an accountability or face-to-name check prior to transitioning from the classroom to the gym.
