Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A renewal inspection was initiated and concluded on 2/2/2022. The Director was contacted by telephone and a virtual inspection was conducted. There were 37 children present, ranging in ages from 5years to 12years, with 4 staff supervising. The Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of 3 children's records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and one violation was documented on the violation notice issued to the program.

Based on a review of children's medications, it was determined that two authorization forms are not complete.
Evidence:
Two medication authorization forms for Child C do not include the parent's signature.
Plan of Correction: I will ask the parent to sign both forms.
