Inspector Notes

An unannounced licensing inspection was conducted today. There were 54 children in care at the time, with a total of 12 staff members assigned to their direct supervision. The children were engaged in outdoor time, classroom tabletop activities, creative play, and group sports activities during the inspection. The children were served chicken wraps, celery with ranch, peaches, and milk for lunch today.
The Inspector reviewed compliance in the areas of nutrition, programming, the physical plant, administrative records, children records, medications, and staff training.

Standard 8VAC20-780-130-A
Based on a review of children's records, it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
In child record D there is no documentation of an immunization record. This child entered care on 6/5/2022.
Plan of Correction: The parent will be reminded to bring the immunization documentation information.
Standard 8VAC20-780-140-A
Based on a review of children's records, it was determined that the center has not obtained documentation showing that each child has received a physical exam under the direction of a physician within 30 days of enrollment.
Evidence:
Child D entered care on 6/5/2022. There is no documentation of a completed physical exam on file for this child.
Plan of Correction: The parent will be reminded to bring the child's physical exam.
