Inspector Notes

An in-person focused monitoring inspection was conducted to investigate supervision on July 13, 2022. Interviews were conducted and documentation was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-340-F
Based on interviews, the center staff did not have actual sight and sound supervision of children under 10 years of age.

Evidence:
1. At about 8:45am on June 2, 2022, Staff 1 was lining up children to return to the classrooms after playing in the gym. Staff 1 stated they saw Child 1 in the back of the line during the transition back to the classroom. Upon returning to the classroom, Staff 1 immediately went to use the restroom leaving the children in the care of Staff 2. A few minutes later, Staff 2 stated that Child 1 came to the classroom and stated Staff 1 left them in the gym. Staff 1 and 2 stated the child was left in the gym and walked back to the classroom by themself. Staff 1 and 2 stated Child 1 was left unattended for no more than 5 minutes.
2. Child 1 stated they came out of the bathroom located in the gym and ?no one was in the gym.? Child 1 walked back to the classroom and told Staff 2 that they were left alone in the gym. Child 1 stated they were alone for a ?little bit of time.?
Plan of Correction: Bathrooms will be checked and cleared of children before transitioning children.

Based on interviews and documentation reviewed, the center did not follow their own policies and procedures regarding supervision and accountability of children.

Evidence:
1. The School Age Child Care Staff handbook states that ?all children must be accounted for at the beginning and end of the transition time through the use of the 4-part accountability system (passports, locators and accountability checklists) and center specific procedures.?
2. Staff 1 stated a headcount was not conducted at the end of the transition from the gym to the classroom.
Plan of Correction: Maintain sight and sound at all times. We will do headcounts and use checklist to make sure all children are accounted for before doing any transitions
