During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on April 24, 2025 and completed on April 24, 2025, as a part of the current licensure period. The on-site inspection began at 7:30am and ended at 8:30am. The inspector reviewed compliance in the areas listed above. There were 14 children present and 3 staff. The inspector reviewed 5 child files and 3 staff files on-site. This inspection included: document review, postings review, tour of the facility and playgrounds, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Name, home address, and home phone number of each parent as well as, work phone number and place of employment of each parent who has custody.

Child A did not have parent information on file.
Plan of Correction: Information will be obtained from parent at pick-up today.
