Inspector Notes
An unannounced monitoring inspection was conducted on 11/21/2023 from 8:40 am-9:22 am. There were 19 children supervised by 3 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play and tabletop activities. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. 2Based on record review and interview, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
