Inspector Notes

An unannounced monitoring inspection was conducted on 11/21/2023 from 8:40 am-9:22 am. There were 19 children supervised by 3 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play and tabletop activities. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. REPEAT VIOLATION

Based on record review and interview, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
1. There was no documentation of an immunization record for Child #3.
2. The site supervisor confirmed Child #3's start date was October 2023.

REPEAT VIOLATION

Based on record review and interview, the center did not obtain all the required information for each child's record.

Evidence:
1. Child #4 is missing the names, addresses, and phone numbers of two emergency contacts.
2. Child #5 is missing the addresses for two emergency contacts
Plan of Correction: Get mom to fill in one contact even if just a neighbor and for other emergency contacts I will get parents to add two.
