Inspector Notes

An unannounced monitoring inspection was initiated on 4/29/2025 and completed on 5/1/2025 in response to a self-report received by the licensing office on 4/24/2025 relating to supervision. The inspector reviewed 1 child?s record on-site and 2 staff records on-site. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/7/2025. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On April 22, 2025, at approximately 5:30 pm, Child #1 (age 7 years old) was left outside on the playground without actual sight and sound supervision for approximately five minutes. During transition, Staff #1 and Staff #2 called for the class to line up, counted seven students, and left the playground through door #5. They entered classroom #2 for handwashing and transitioned the students to classroom #1, where Staff #3 was present. While students were transitioning, Staff #3 and Staff #4 heard the doorbell at door #3 and saw Child #1, who had been left on the playground, walking into classroom #1 behind two parents. Staff #1 reported that one of the parents who walked in with Child #1 stated Child #1 was outside looking for SACC.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

On April 22, 2025, Staff #1 and Staff #2 did not follow the proper accountability procedures stated in their Safety and Accountability Plan. It is documented that staff are required to complete face-to-name checks using the accountability list, stopping at a checkpoint to recount students, and staff are to walk around the playground for a final check before going inside. As a result of the procedures not being followed, Child #1 was left unsupervised for approximately five minutes.
