Inspector Notes

An announced review of staff personnel files was conducted on 1/23/2025 and completed on 1/23/2025 by inspectors Diana DeSilva, Miranda Wright, Ariel Hayes and Margaret Allworth at the Pennino SACC office building. The inspection began at 9:30 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. The inspection included document review.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A tuberculosis (TB) screening is required every two years from the last screening.

Staff #3 updated screening was due 5 days ago. The updated screenings have not yet been completed.
Plan of Correction: TB test was scheduled by staff member.
