Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/8/2025, as a part of a self-reported incident regarding Supervision. The on-site inspection began at 12:00pm and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There was 1 child present and 78 children away on a local field trip and there were 3 staff present and 17 staff away on the local field trip. The inspector reviewed 3 children?s records and 5 staff records on-site and electronically on 7/8/2025 and 7/10/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/22/2025. A POC submitted after this date will not appear on the public website.

The center is required to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #1 and Child #3 did not have documentation of immunizations on file at the time of the
inspection.
Plan of Correction: The parents were notified to provide the required documentation for their children's files.
Standard 8VAC20-780-240-E
Within 30 days of the first day of employment, staff must complete orientation training in first
aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in
care.

Staff #2 did not have documentation on file of current first aid and
cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care at
the time of the inspection.
Plan of Correction: Staff were informed that they need to sign up for the required trainings.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.

At around 4:50pm on June 26th, Staff #2 was walking 5 children from the Exploration room (117) to the Main SACC room (135) when Child #2 and Child #3 went back to the Exploration room to get their backpacks. Staff #2 thought that Staff #5 was still in the Exploration room cleaning up, so allowed the children to go back on their own. When Child #2 and Child #3 went to the Exploration room, there were no staff members in the room. Staff #2 had already made their way with the remaining group of children to the Main SACC room which was down the hall and around the corner from where the Exploration room is. Staff #4 was standing near the SACC entrance and saw Child #2 and Child #3 come around the corner from the Exploration room without a staff member. Staff #5 returned Child #2 and Child #3 to the Main SACC room where Staff #2 was. Child #2 and Child #3 were without site and sound supervision for approximately 2 minutes.
Plan of Correction: The safety and supervision procedures were reviewed with the staff involved.
