During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous
inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If
there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/01/2025. The on-site inspection began at 12:40pm and ended at 1:26pm. The inspector reviewed compliance in the areas listed above. There were 40 children present and two staff. The center was over ratio until the third staff arrived at approximately 12:46pm The inspector reviewed
four children?s records and three staff records on-site on 4/01/2025. This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/08/2025.
A POC submitted after this date will not appear on the public website.

The required ratio for children ages School age eligible up to 9 years is one staff per 18 children.

Staff #1 and Staff #2 were supervising 43 (School age eligible) children from approximately 10:30am-11:30am and 41 children from approximately 11:30am-12:10pm 40 children from approximately 12:10pm-12:46pm until a third staff person arrived..
Plan of Correction: Substitute teachers will be provided when staff are out on leave. Other SACC Centers will lend staff when possible.
