63.2 Child Abuse & Neglect
Inspector Notes
An unannounced renewal inspection was conducted between 2:45pm-4:45pm today. There were 28 children present and four staff members supervising. Children were observed hand washing, eating snack, and transitioning to the playground. The physical plant, programs, children's records, medications, and emergency drills were also reviewed. Staff records are reviewed annually in the administrative office; however, Medication Administration Training (MAT) and First Aid and CPR certificates were available for review.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
4Evidence:
1. Staff #2 did not wash their hands before serving food or beverage. Staff #6 put on food service gloves, but did not wash their hands before serving children snack and beverage.
Evidence:
1. Child #6 has emergency medication on site that does not have the pharmacy prescription label attached.
Evidence:
1. According to an injury report dated 10/18/2021, Child #6 jumped off of a jungle gym located on the playground and hurt their right wrist. Child #6 was taken to the doctor's office where they were diagnosed with a "Bone Buckle" and received a cast for the right arm.
2. The department's representative was not notified of this injury within two days.
Evidence:
1. The findings of the most recent inspection of the facility was not posted in a location to be easily read. The violation notice was located in a folder labelled "Licensing", but the violation notice was turned towards the wall and not revealing the contents.
