Inspector Notes

A subsidy health and safety (SHSI) inspection was initiated on 11/22/21 and concluded on 11/22/21. There were 24 children present with 4 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records, one staff record did not include documentation of completing the required pre-service training (10 hours) within 90 days of employment.
Evidence:
The record for Staff #1 did not include documentation of the 10 hour pre-service training within 90 days of employment. (Date of Employment: March 2020) Staff began working in September 2020, when schools reopened. The course was not completed until October 2021.
Plan of Correction: We will work to ensure that all staff have the training completed in the appropriate time frame.

Base on a review of records, staff records are incomplete.
Evidence: 2 of 4 staff records did not contain annual documentation of the Virginia update Health and Safety training. Documentation available for review was dated 2019, and 2021. Training should have been completed in 2020 as staff were working at that time. (Staff #2, 3).
Plan of Correction: We will work to ensure that all staff have the training completed in the appropriate time frame.
