Inspector Notes

An investigation was begun on 12/7/2022 in response to a self reported incident being received in our office regarding a lack of supervision.
01/09/2023 - An in person inspection was conducted today after school. Children were observed washing hands, eating snack, arriving from other programs, and enjoying free play activities.
1 staff record was reviewed for specific items only, as the entire record is reviewed at the main office at another time. 2 child records, emergency evacuation drills, medications, first aid supplies and age appropriate activities were all reviewed/observed.

Evidence:

Based on multiple staff statements, a child 10 years of age and older was not kept in actual sight and sound supervision.
Evidence:
On 12/05/22 at approximately 4:40 PM a child was left on the playground, when the group of children and staff they were with left the playground to go inside. The playground area is located up an exterior flight of stairs and behind the school building. The child was found by a different staff member at approximately 4:45 while retrieving another child from a school bus arriving at the facility, Staff were unaware that the child had not left the playground with the group. Based on multiple staff statements, the center failed to follow the policy for knowing where children are at all times.
Evidence:
On 12/05/2022, Staff A did not follow the SACC policy for accountability when they left the playground area to go into the SACC rooms with a group of 16 children. As the group prepared to go into the SACC rooms, the children started down the stairs with out a roster check/head count being conducted as a second staff member arrived.
