Inspector Notes
An unannounced monitoring inspection was conducted at 10:50am. Observed 20 children + 3 direct-care staff. Ratios and supervision were in compliance. Children and staff from 2 different schools were present today. Children were observed having pajama day and watching a movie. The classrooms were clean and well supplied with equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Standard 8VAC20-780-130-A
Based on records reviewed, it was determined that 1 of 2 children's records did not include documentation of immunizations. Evidence: The record for Child #6 did not include immunizations. The child attends another nearby school and access to health records is not at this location.
Plan of Correction: The center supervisor for the location where the child attends school and SACC was made aware of the missing immunizations and said the record would be obtained.
Standard 8VAC20-780-140-A
Based on records reviewed, it was determined that 1 of 2 children's records did not include documentation of a physical exam. Evidence: The record for Child #6 did not include a physical exam. The child attends another nearby school and access to health records is not at this location.
Plan of Correction: The center supervisor for the location where the child attends school and SACC was made aware of the missing physical exam and said the record would be obtained.
Standard 8VAC20-780-280-B
Based on observations made, it was determined that a hazardous substance was not kept in a locked place. Evidence: A can of spray disinfectant was found to be sitting on a storage container located in the bathroom used by children in one classroom. The product was sprayed and determined to have disinfectant in the can.
Plan of Correction: The spray disinfectant was disposed of during the inspection.
