Inspector Notes

Conducted an unannounced monitoring inspection at 12:00pm. Observed 16 children + 2 direct-care staff. Ratios and supervision were in compliance. SACC programs from Riverside and Woodley Hills were combined today. Children were observed finishing lunch, playing games and with tablets/electronic devices. Children brought their own lunches today. The facility was found to be clean and well supplied toys and equipment for the children. Repeat violation.
Based on records reviewed, staff failed to ensure that 1 of 2 children's records included required information. Evidence: The record for Child #7 did not include documentation of immunizations prior to the child attending. To-date immunizations were not on-file.
Plan of Correction: Ask the school or parent to provide a copy.
Standard 8VAC20-780-40-E
Based on records reviewed and information obtained from SACC administration, staff failed to follow the medication policies/procedures by not ensuring that an emergency medication was on-site where the child was in care.

Evidence:

1. A medication authorization and allergy action plan belonging to Child #7 (attending from SACC Woodley Hills) indicated that a prescription emergency medication be given in an emergency but the medication was not on-site, as required.
2. Staff #8 reported the medication to be in the nurses office at Woodley Hills ES and not accessible by SACC staff today.
Plan of Correction: Request medication from parents.
