During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 11/19/24, as a part of the provisional licensure period. The on-site inspection began at 3:59pmand ended at 4:39pm. The inspector reviewed compliance in the areas listed above. There were 34 children present and 4 staff. The inspector reviewed 3 children?s records and 1 staff record while on-site. This inspection included document review, tour of the program areas and interviews.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/28/24. A POC submitted after this date will not appear on the public website.

[Note: 8VAC20-780-(2)-140.D] Pursuant to the Code of VA, physical exams are not required for any child whose parents objects on religious grounds. The parent must submit a signed statement noting that the parent objects on religious grounds and certifying that to the best of the parents knowledge the child is in good health and free from communicable or contagious disease.

The religious exemption signed by the parent of Child #6 referenced immunizations, but made no refence to a physical exam and did not state that the child is in good health and free from communicable or contagious disease.
Plan of Correction: Center staff will ask the parent to proved an updates/revised religious exemption statement or a copy of the child's school entrance health form. 11-25-24
Once received the document will be placed int he child's file.
