Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/3/2024 and completed on 10/3/2024. The on-site inspection began at 10:00am and ended at 10:55am. The inspector reviewed compliance in the areas listed above. There were 14 children present and 3 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each center shall maintain and keep at the center as separate record for each child enrolled while shall contain the work phone number of each parent who has custody.

The center did not have the work phone number of each parent who has custody of Child #1, Child #3, and Child #4.
Plan of Correction: The center will obtain the missing work phone number of the parents.
