Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/03/24 and completed on the same date, as a part of licensure period. The on-site inspection began at 12:45 and ended at 2:00. The inspector reviewed compliance in the areas listed above. There were 31 children present and 4 staff. The inspector reviewed 5 children?s records and 2 staff records on site. This inspection included, document review, tour of the facility, Interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program .

Standard 8VAC20-780-60-A
A child's record must contain the following information: work phone number and place of employment of each parent who has custody and name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached among other information.

One child, child 1, is missing the parents work address and work phone numbers, and address and phone number for the second emergency contact.
Plan of Correction: We will ask the parents for the information.
