Inspector Notes

A monitoring inspection was conducted on 10/17/2023. There were a total of 32 children with 3 staff members present. Children were observed arriving to the program and engaged in various activities including legos, and art. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today in order for it to be included on the website. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on observation, the center did not ensure medication was labeled with the child's name, dosage amount, and the time or times to be given.
Evidence:
Child #6 had medication on site that was not labeled with their name or dosage amount.
Plan of Correction: A new amount of medication has been obtained with their name and dosage amounts.

Based on a review of staff records, the center did not ensure the required information was in each staff's record.
Evidence:
Staff #1 did not have emergency contacts documented on site during the inspection.
Plan of Correction: Staff #1 now has emergency contacts documented on site.
