Inspector Notes

A monitoring inspection was conducted on 03/08/2024. There were a total of 28 children with 3 staff members present. Children were observed arriving to the program, eating breakfast, and engaged in many activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today in order for it to be included on the website. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard 8VAC20-780-245-J-1
Based on a review of records, staff records do not contain documentation of having completed the Medication Administration Training (MAT) course with medication present at the center.
Evidence:
3 of 3 staff records were reviewed. None of the 3 records contained documentation of current certification in MAT training, with emergency medications present at the center. MAT training for staff #1 expired on 03/06/2024.
Plan of Correction: Not available online. Contact Inspector for more information.
Standard 8VAC20-780-245-J-3
Based on a review of staff records, the CDC did not ensure children whom had emergency medications and epinephrine were in the care of a staff member who meets the requirements.
Evidence:
Staff #1 MAT training expired on 03/06/2024. 3 Children onsite had epinephrine prescribed in their allergy action plan.
Plan of Correction: Center supervisor has signed up for and has begun working on MAT training renewal and will be completed April 11.
Standard 8VAC20-780-40-M
Based on a review of documentation and staff interview, the CDC did not maintain a current written list of all children's allergies, sensitivities, and dietary restrictions.
Evidence:
There was no allergy list documented for 3 children that had emergency medication on site. The director confirmed they did not have a written allergy list available.
Plan of Correction: Center supervisor and staff will work to put together an updated list of youth that have allergies, sensitives and dietary restrictions.
Standard 8VAC20-780-550-G
Based on a review of the emergency evacuation log, the CDC did not ensure documentation was maintained of emergency evacuation drills.
Evidence:
The last documented emergency evacuation drill was dated 12/24/2023. Documentation for January 2024 and February 2024 was not maintained.
Plan of Correction: Center supervisor has updated the emergency evacuation log and will ensure that it is current. Evacuation dates updated by center supervisor.
