Inspector Notes

An unannounced, on-site monitoring inspection was initiated on September 13, 2024 and completed on September 13, 2024, as a part of current licensure period. The on-site inspection began at 7:55am and ended at 8:45am. The inspector reviewed compliance in the areas listed above. There were 25 children present and 3 staff. The inspector reviewed 4 children?s records and 3 staff records on-site.

This inspection included:
? document review of Injury logs, policies and procedures, emergency drill logs, medication
administration logs, authorization forms,
? tour of the facility,
? interviews,
? observations of the children playing board games, reading, interacting with the staff.

Information gathered during the inspection determined non-compliance with applicable standards or
law, and violations are documented on the violation notice issued to the program.

Based on record review of 4 child files, it was determined that the center did not ensure that the separate record for each child enrolled contained all of the required information.

Evidence: The record for child #2 did not contain documentation of work phone numbers for each parent.
Plan of Correction: We will get complete parent information for all children going forward.
