63.2 Child Abuse & Neglect

During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on November 18, 2024 and completed on November 19, 2024 in response to a self-report received by the licensing office on November 15, 2024 relating to supervision. The inspector reviewed 6 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to ratios and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 26, 2024. A POC submitted after this date will not appear on the public website.

Children 10 years of age and older shall be within actual sight and sound supervision of staff.

Child #1, age 12, was not within sight and sound supervision on November 6, 2024, for approximately 1-2 minutes after being sent to the front of the school alone to get children from a bus.
Plan of Correction: -Staff will ensure all children are within sight and sound supervision at all times. A staff will be assigned to receive children off the bus to ensure appropriate supervision is maintained.
-The Regional Supervisor will review supervision policies and procedures with all center staff.

The required ratio for children school age eligible up to 9 years old is one staff per 18 children.

Staff #1 was observed on November 18, 2024, taking 17 children (youngest child 8 years old) to the front of the school to get children arriving from another school from the bus, totaling 21 children. Staff #1 was over staff to children ratio by 3 children for approximately ten minutes until they all were taken to the playground where the other staff were.
Plan of Correction: Appropriate staff to child ratios will be maintained at all times among each grouping of children.
