Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the facilitty.

A subsidy health and safety (SHSI) inspection was conducted on 9/30/2021. There were 24 AS children enrolled present with 4 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff statement, no written allergy care plan is available.
Evidence:
Child #1 does not have a written allergy care plan that has instructions, from a physician, regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Plan of Correction: An allergy plan will be given to the parent to take to the doctor for the written documentation.

Based on staff statement and record review, all staff do not have current first aid and CPR certification.
Evidence:
Staff #2's certification expired 5/2021 for both First Aid and CPR.
Staff #4's first aid certification expired 9/24/2021.
Plan of Correction: Trainings are being scheduled for November 2021.
