Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 01/08/2026 and completed on 01/08/2026. The on-site inspection began at 7:25AM and ended at 8:05AM. The inspector reviewed compliance in the areas listed above. There were 2 children present and 3 staff at the beginning of the inspection. The inspector reviewed 3 children?s records and 3 staff records on-site on 01/08/2026. This inspection included document review, a tour of the facility, interviews and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-80-B
Centers shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Child 1 received outside medical treatment for an injury sustained while in care at the center on 12/01/2025. The injuries were noted on an email from the facility to the administrative office at 9:13AM on 12/1/2025, pending review, before the Department was not notified on 12/5/2025.
Plan of Correction: Injuries requiring outside medical attention will be reported to the department within 48 hours.
